AAMS Payment Policies
Payment is due at time of service. Insurance forms are available upon request. Appointment cancellations MUST be made at least 24 hours prior to your appointment to avoid a $50 cancellation fee.

Privacy Notice
Office visits are absolutely confidential. Our office complies with HIPPA. Under no circumstances (other than a direct court order by a judge) will records be released without written permission of the patient.
